Part 4: Project Planning and Implementation Templates

You can adapt each template to include new questions relevant to your community.

The templates are named according to the activity and relate to the steps outlined in Part 1: Ten Steps to Planning Your Health Promotion Project.

The templates provided are:

 

1. Research: The research template gives you a list of questions you can use to start researching the health needs of your community.

5. Plan Writing :  
The plan writing template helps you to develop the goals, objectives, strategies and activities of your project.

6. Key Messages: The key messages template helps you to start creating the messages you want people to know.

7. Resourcing: The resourcing template helps you to prioritise the resources you will need to run your project.

13. Media Plan: The media plan template helps you to plan to work with local media outlets to promote your project.

16. Permission Form: The permission form template helps you to create a consent form that participants in your project should sign before you take photos of them or film them.
